The following forms are intended for members of the Msgr Dewson Council of the Knights of Columbus.
- Charitable Request Form - (PDF, 283 KB) To request support from the Knights of Columbus, a member should fill out this form and bring it to the next KofC meeting for submission to the Council.
- Member Interest Form - (PDF, 26 KB) Members can fill this out to inform the Grand Knight and the Council what activities they would be interested in participating in.
